Doctor McClelland's autopsy notes
McClelland didn't see the back wound because doctors were working on Kennedy while he was lying on his back.
Note that the wound in Kennedy's back is lower than the wound in his throat, which means there were shooters behind and in front of the president. [Later: That is, those were shots from two different shooters]
